Preparing For Your Appointment

    • Please fill out the new patient packet and intake form on the forms page here

    • If assistance is needed to provide your history, prepare to ask a family member or close friend to come with you for the interview portion of the evaluation

      • The family member or friend is free to go after the clinical interview, which takes approximately 30 minutes

    • If possible, collect records of past neurodiagnostic testing (e.g., CT or MRI scans) and/or results from previous neuropsychological evaluations conducted at other practices, hospitals, or institutions

      • These would be beneficial for the assessment process

    • Please get a good night’s sleep and eat breakfast, as evaluations can take 2-5 hours long

    • Do not consume alcohol or any recreational drugs starting one day before your appointment

    • Please be aware of our office policies, you can read them in the new patient packet here or on our policies page here.

    • Please bring a list of current medications, and any assistive aids such as glasses, hearing aids, walkers, etc.

    • If you have not filled out the new patient packet and/or the intake form, then please arrive 15-20 minutes early to fill it out

What to Expect During Your Appointment:

Your testing day, also known as an assessment, generally takes between two and five hours to complete, but can take up to eight hours, depending on the complexity of the issues to be addressed by the evaluation and your conditions (for example, fatigue, confusion, and motor slowing can extend the time required for an evaluation). Occasionally, it may be necessary to complete the assessment over two or more sessions.

Each assessment is customized to your needs and developmental level. There will be series of interviews and tests (some administered on paper or computers). Some tests may be may be easy, while others may be complex, but the most important thing is to try your best. There are no right or wrong answers and it’s important to try your best.

What to Expect After Your Appointment:

After your assessment/testing day, it takes a minimum of 21 business days for Dr. Hopewell to evaluate, process and send your report. Follow-up appointments are optional, but are recommended if you were not referred by a physician or need a a more detailed explanation of the evaluation report and its recommendations.
