All patients must fill out both the “Adult Intake Form” and the “Consent Form” prior to being tested.
The Process
Day/Night Before Testing
Please try to get a goodnight sleep
Please bring your completed paperwork, list of current medications, and all assistive devices (this includes glasses, hearing aids, walkers, etc.) to your scheduled appointment
If you did not complete the paperwork, please come 15-30 minutes early to complete it in our office
Day of Testing
Please eat prior to coming to our office
You will have a clinical interview with one of our clinician and will be tested afterwards with one of our Psychometrists
After Testing
You will be directed by Advantage Healthcare Systems on your next steps
It takes 21 business days for Dr. Hopewell/HNC to send Advantage Healthcare Systems a copy of your report
